HOURS OF OPERATION 

Monday, Tuesday, & Friday: 7 a.m. – 6 p.m.

Wednesday(Christmas Eve & New Years Eve):7 a.m. – 12 p.m.

Thursday(Christmas Day & New Years Day): CLOSED 

 

 Parents/Guardians requiring assistance with online registrations may call our School Age Services staff for online enrollment instructions @ 417-781-9622.

  • Current Working Phone Numbers
  • Primary & Secondary Parent/Guardian Information
  • 2 Emergency Contacts

Use your back arrow button when available during the enrollment process instead of “X.”

Please Note:  Registrations will not be considered complete without the Documents listed below:

  1. The Medication Authorization Form (To be completed by parent/guardian)
    • Required fields must be completed:
      • Child’s Name, Parent/Guardian Signature, and Date
  2. If applicable:  Child’s IEP or Specialized Care Plan

Forms, records, and documents where applicable, may be submitted using one of the following options:
