After School Program


Parents and/or Guardians can call us at 417-781-9622 and ask for School Age Services Staff to get set up for online enrollment!

  • Info to gather before you begin:  current working email addresses, phone numbers, and addresses for a minimum of 2 emergency contacts in addition to the primary & secondary parent or guardian information. 

  • Use your back arrow button when available during the enrollment process instead of “X.”

Please Note:  Registrations will not be considered complete without the 4 State Required Documents listed below:

  1. The Medication Authorization Form (To be completed by parent/guardian)
    • Required fields must be completed:
      • Child’s Name, Parent/Guardian Signature, and Date
      • If your child is not on any medication, please type “No Meds”
  2. The School-Age Child Health Report  (To be completed by parent/guardian)
  3. Immunization Records
  4. If applicable:  Child’s IEP or Specialized Care Plan

Forms, records, and documents where applicable, may be submitted using one of the following options:

    • Upload Forms to our secure link and send through SharePoint by clicking here
    • or Provide Forms in-person to our facility
    • or FAX Forms to us at 417-625-2503

Automatic bank and credit card drafts for the enrolled child’s tuition are processed weekly every Monday, regardless of bank holidays.
