Students currently enrolled in our After School Program or students who attended our Thanksgiving or Winter Camp can register online at joplinfamilyy.org

Students not currently enrolled:  Parents/Guardians may get students registered by calling our School Age Services staff for an access code for online enrollment @ 417-781-9622.

INFO TO GATHER BEFORE REGISTERING:

  • Current Working Phone Numbers
  • Primary & Secondary Parent/Guardian Information
  • 2 Emergency Contacts

Use your back arrow button when available during the enrollment process instead of “X.”

Please Note:  Registrations will not be considered complete without the Documents listed below:

  1. The Medication Authorization Form (To be completed by parent/guardian)
    • Required fields must be completed:
      • Child’s Name, Parent/Guardian Signature, and Date
      • If your child is not on any medication, please type “No Meds”
  2. The School-Age Child Health Report (To be completed by parent/guardian)
  3. If applicable:  Child’s IEP or Specialized Care Plan

Forms, records, and documents where applicable, may be submitted using one of the following options:

    • Upload Forms to our secure link and send through SharePoint by clicking here
    • or Provide Forms in-person to our facility
    • or FAX Forms to us at 417-625-2503
